Field positions

Ideal candidate will
Supervise assigned nursing and paraprofessional personnel in the delivery of services to patient clients and families
Extensively interacts with patient clients and families monitoring and ensuring the quality of services provided
Proactively seeks way to improve client satisfaction
Participates in the coordination and management of all services rendered to clients and families by office staff and per Diem employees under his/her supervision
Provides on-site supervision at start of care
• Participates in direct service staff selection, orientation and in-service education.
Provides the direct evaluation of the clinical competence of assigned personnel and participates with appropriate supervisory staff in the evaluation of other direct service staff
Participates in the development of company's objectives, standards of care, policies and procedures affecting clinical services
• Travel to client's homes using own vehicle
Participates in the planning and implementation of caregiver evaluation, orientation and other in-service educational program
Participates in the development and implementation of standardized clinical policies and procedures

Minimum Qualifications:
Registered Nurse, BSN preferred
RN License in the state of New York
Experience in Home Care Case Management or Hospice or Acute Care preferred
Private pay experience a plus
8 years of full time clinical experience in nursing
Valid Driver's License
